To Whom It May Concern:
It is with a heavy heart and immense sadness that I must let you know that my health has forced the closing of Crawford-Green Cardiology Group. Starting November 1, 2023, our doors will officially close. For nearly 25 years, I have attempted to deliver the most compassionate care to each of you. My life has been greatly enriched by these experiences. I regret that my health has forced this painful decision. As always, I wish you the best as you seek medical care from other providers. I shall miss you all.

Prescription Refills
If you need a prescription refill, please notify the office no later than Friday, October 27, 2023.
